Registration and Request for Childcare
The Cleft Palate Foundation is pleased to present Connections 2008: A Conference for Patients and Families on Saturday, April 12, at the Loews Philadelphia Hotel, Philadelphia, Pennsylvania. Care and activities for children will be available throughout the day.
All registration fees include programming, an event t-shirt, snacks and lunch. Registration for one person is $30.00; each additional person is $20.00. For children 12 and under, the fee is $10.00 and includes childcare.
Online registration is now closed. Please either print off and complete a hard copy of the registration form and bring the form with you to the Conference or feel free to come to the Conference on Friday night or Saturday morning to register on site.
The ACPA/CPF room block at the Loews Philadelphia Hotel is now full.
